Dear Vice President Customer Service / Agent:
Enclosed/attached;  please find supporting copies for the claim I will mention next.
On may 24th of this year, 2011 I placed two orders from Amazon.com. one for the PRIME MEMBERSHIP trial and another for a Garmin 1350 factory refurbished GPS. I did not receive the GPS until June 8th. This was caused by several different issues which I will explain.
If you will note your records of my personal profile for the date in question, if your records hold time stamped changes, you will find that I had and still have three addresses listed there. 1) my billing address, 2) my postal address and 3) my physical address. As you may be aware, Your order entry data entry screens do not specify if a shipment will be delivered via USPS, UPS or FEDEX. As of the date of order and possibly to this date, your order entry and shipping confirmation software does not check for or flag miss-directed shipments for customer correction prior to shipping. And further, even if somewhere in your shipping and ordering policies it is stated that your 2 day shipping will be sent UPS, it seems only prudent that your confirmation software should flag undeliverable addresses.
On may 26th  I contacted Jyothi Dara at customer service by telephone after submitting a customer service request by Amazon memo for same. After about 30 minutes she thought she understood what had transpired and had corrected the situation. Several days latter I contacted UPS And discovered that the item was being returned and I would have to reorder.
On June 8, my credit/debit card account was credited so that I could reorder the GPS. I had started the reorder on June 4th with the customer service agent, Karol S. At that time she assured me that my order would be sent out next day air free of charge and that my PRIME MEMBERSHIP would be canceled on that date. Well it appears that neither of those things happened as you can plainly see in my Amazon account records. I found out what had not been done as promised by receiving a note from my personal banker.
Unfortunately I do not have all the time in the world to gather every bit of info regarding this situation. But be rest assured that I can. I saved all the written correspondence and telephone conversation notes some where in my laptop.. I should not even be spending time to write this letter. I do not believe that any of this is of my doing. The only way I can see that I may have avoided these happenings is to have ORDERED FROM ANOTHER COMPANY. And that would be a shame as Amazon and I have enjoyed doing business for some years now! I would like to think we could  continue this relationship for some time to come…
So, to recap;
I Placed order on May 24.
Order was returned on or about May 28
 June 4th I got the okay to reorder my GPS, a promise of next day air delivery and to cancel the PRIME MEMBERSHIP.
On June 8 my Credit card is credited so I may complete the purchase.
Delivery on the 10th of June.
June 27th billed for $79.00 for Prime Membership.
June 28th charged a $35.00 overdraft fee for the Prime Membership charge.
July 2nd I canceled the Prime Membership.
Today Thursday, July 14, 2011, I am making my first formal request for compensation for bank charges that through no fault of my own have been made against me that are directly related to an action or lack of action by your agent, namely, Karol S. The remedy that I am quietly demanding is payment of the $35.00 overdraft fee caused by the erroneous charge and a letter explaining and requesting forgiveness to my personal banker on my behalf at your earliest convenience no later than seven day after receipt of this request.
I am sending this letter by Amazon memo or Email and another copy by USPS to Amazon Corporate Headquarters, Attention VP of Customer Service.

Sincerely,
NICK C TILBERG
